Personal Emergency Evacuation Planning Checklist

The Personal Emergency Evacuation Planning Checklist is provided as part of the Emergency Evacuation Planning Guide for People with Disabilities. This checklist can be used by building services managers and people with disabilities to design a personalized evacuation plan. To personalize the form, and add the appropriate information specific to the person for whom the plan is being built, download it to your local hard drive, then copy and rename the file for each individual for whom an evacuation plan is needed. The file name will print in the footer of each page, so use short, meaningful file names, for example, MaryEvacPlan.doc.
